Find the educational activity you attended from the list below, click on the associated link and you will be taken to the survey you need to complete to record your attendance and receive continuing education credit.

Upon completion of the survey, you will be asked for your legal name as it appears on your professional license because this is how it is recorded in our system. For providers, we will also need your NPI number as a unique identifier to ensure accurate recordkeeping as we have providers with the same or similar names.
