It is helpful to have the following information available:
- Patient name, date of birth and contact information
- Recent medical history and physical
- Recent echocardiogram and cardiac catheterization
- Most recent ejection fraction
- For patients with aortic stenosis: The aortic valve area and mean pressure gradient and date of measurement
- For mitral patients: RV systolic pressure and LV end systolic pressure
- Your contact information
