Addressing Barriers to Social Drivers of Health

The Institute for Medicaid Innovation highlights that socioeconomic and physical environmental factors contribute to 50% of our overall health, while another 30% is influenced by our health behaviors. Only 20% of our health outcomes are determined by access to quality healthcare services. By helping patients overcome these barriers, we can have a significant impact on improving healthcare across the state.
