Empowering You

Empowering You
Bringing resources close to home
At Piedmont, we know that health is influenced by much more than medical care—it is shaped by the social, economic, and environmental factors that affect daily life. We are committed to ensuring every patient has access to the resources and support needed to achieve their best possible health outcomes. Through our Piedmont-based programs and our network of community-based organizations (Empowering You), we strive to reduce barriers, improve well-being, and build healthier, stronger communities. Together, we can create a future where care extends beyond the clinical setting and promotes lasting health for all.

Addressing barriers to social drivers of health
The Institute for Medicaid Innovation highlights that socioeconomic and physical environmental factors contribute to 50% of our overall health, while another 30% is influenced by our health behaviors. Only 20% of our health outcomes are determined by access to quality healthcare services. By helping patients overcome these barriers, we can have a significant impact on improving healthcare across the state.
