Bariatrics Virtual Support Group Consent
In order to participate in the Telephonic Support Group at Piedmont Atlanta you must agree to all of the “Rules for Attending” and sign below. You can expect to receive login information for each support group a few days prior to the event. Please note that the login information does change for each group.
At this time, this group is reserved for patients of Piedmont Healthcare.
Rules for Attending
- Always announce yourself upon entering the call
- Only use your first name for introduction
- Understand there are other bariatric surgery patients on the call
- Do not use the telephonic support group to replace medical, psychological, or nutrition treatment or counseling
- Remember to allow everyone an equal opportunity to speak
- Do not share personal medical information on the call
- Do not record the call
- Please let the leader know upon start of the call if you will be leaving the call early
- I understand that everything said on the call is confidential. I agree not to disclose the names or any information shared by other members of the group.
