The Piedmont personal health assistant program eliminates the traditional administrative burdens of health care by providing enhanced scheduling support, proactive outreach, and a single point of contact for any questions or barriers you may have. We designed it to save time and keep the health and wellness of participants and their families on track. We are committed to helping patients achieve their health and well-being goals.
 
Your health assistant serves as your single point of contact for care coordination and questions:
• Schedules appointments across the Piedmont network for various specialties
• Provides upcoming care reminders & pre-appointment instructions

Tailored experience

Your health assistant will tailor communications and services to your preferences:They’ll communicate in a way that works for you, your schedule and your lifestyle (email, text, call); they perform time-consuming administrative tasks, e.g., obtaining medical records, handling billing questions.

Personalized care

In addition to scheduling appointments and physicals, your health assistant can: Assist in finding care options that meet your exact needs, such as finding the right provider or specialty; send proactive outreach reminders about annual healthcare services (annual physicals, mammograms, flu shots, etc.).

Membership options

We offer base and family membership options. Each is personalized to you and can be the complete roadmap of services for your family’s needs. Base membership: $250 per patient, per year; Family membership: $400 per family, per year (up to 5 family members).

This program is designed to save you time by assisting with scheduling appointments across the Piedmont network for various specialties, reminding you of your upcoming appointments, and ensuring you have everything you need, so your day of service is a breeze! Your health assistant is personalized to you and your specific needs, or those of your family. The health assistant will conduct a thorough intake call to learn about your medical needs and those of your family (if applicable), and work with you to ensure that everything you need is taken care of, from finding providers to scheduling appointments and addressing any questions along the way.
