A Team Approach to Diabetes Care

Over 8% of the US population — more than 25 million Americans — suffered from some form of diabetes in 2011. As more Americans are diagnosed, we are learning more and more about how this disease affects people in different ways.

One thing that's certain is that almost anyone with diabetes can feel healthier, and live a longer, fuller life with proper management to reduce the risk of developing serious complications.

Managing diabetes is a life-long endeavor. Effective care takes a team effort and a custom approach, like the one offered at Piedmont Healthcare.

Our Diabetes Treatment Team Includes

  • Certified Diabetes Educators
  • Exercise Specialists
  • Physical Therapists
  • Registered Nurses
  • Nutritionists and Dietitians

Our programs are recognized by the American Diabetes Association, and are designed to fit each patient’s individual situation. Our experts will take into account each person’s circumstances, needs and habits to develop a Custom Diabetes Care Plan.

Programs We Offer

  • Personalized instructions
  • Special meal plans
  • Personally tailored exercise regimens
  • On-going follow-up care

The Diabetes Education department provides education and training to individuals diagnosed with Type I Diabetes, Type II Diabetes, Gestational Diabetes, impaired glucose (Pre-Diabetes) and Metabolic Syndrome. Program participants receive nutrition counseling and meal planning to include carbohydrate counting and reading nutrition labels effectively. Participants receive training on blood-glucose monitoring. Instruction is also offered on various insulin delivery systems, as well as symptoms and treatment for hypoglycemia are reviewed.

Successful diabetes management is a team effort. Family members are encouraged to join you in learning to make healthy food choices, developing a plan for regular physical activity, monitoring your blood sugar and learning how and when to take your diabetes medication.
