Life can get pretty hectic and remembering to schedule check-ups, physicals, or follow-ups can take a back seat. Managing unexpected situations and questions can also take time away from the things you love. The Piedmont Personal Health Assistant program eliminates the traditional administrative burdens of healthcare by providing enhanced scheduling support, proactive outreach, and a single point of contact for any questions or barriers you may have. We designed it to save time and keep the health and wellness of participants and their families on track. We are committed to helping patients achieve their health and well-being goals.

 

Your health assistant serves as your single point of contact for care coordination and questions:

• Schedules appointments across the Piedmont network for various specialties
• Provides upcoming care reminders & pre-appointment instructions

 

This program is meant for you to save time by assisting you in scheduling appointments across the Piedmont network for various specialties, reminding you of your upcoming appointments and making sure you have everything you need, so your day of service is a breeze! Your health assistant is personalized to you and your needs or your family needs. The health assistant will do a thorough intake call to learn about you and your family's (if applicable) medical needs and work with you to ensure that everything you need is taken care of from finding providers, to scheduling appointments, and answering any questions along the way.

Will the health assistant provide services for just me or for my whole family?

Under the family plan, the health assistant will provide services for you and your family (up to 5 members). The health assistant will help schedule visits for you and for each additional family member you decide to add to your membership. This includes coordination for adults with Piedmont affiliated physicians. Support may be available for children, depending on geography and the presence of affiliated Piedmont Pediatrics physicians.

Is my health assistant a clinical provider?

No, your health assistant is a non-clinical service focused on administrative / scheduling logistics.

How will the health assistant communicate with me?

During the introduction call, the health assistant will discuss your preferred communication method (phone, email, text).

I am a Piedmont employee and have the Life 360 Health Plan. How will this benefit me?

The Piedmont Life 360 is a clinical care coordination service and Piedmont Personal Assistant is an administrative care coordination service. If you are a Piedmont employee and on Piedmont's Life 360 Health Plan and meet clinical criteria (ie. chronic condition, etc), the health assistant will partner with Piedmont Care Management to ensure you have the highest level of care coordination to help you stay healthy. Are Medicare patients eligible?

Yes. All insured patients are eligible and all insured services will continue to be billed for insurance.
