Services
Our certified oncology nurse navigators serve as a resource, acting as a liaison to enhance the quality and continuity of care and expedite appointments. Oncology nurse navigators’ services are provided free of charge and include:
- Explaining cancer diagnoses, treatment options, and what to expect throughout the care journey
- Assisting with scheduling appointments, coordinating consultations, and facilitating timely access to care
- Serving as a liaison between patients, families, physicians, and healthcare teams to promote seamless communication
- Assessing individual needs and identifying potential barriers to care, including financial, transportation, emotional, or educational concerns
- Connecting patients and families with community resources, support services, and cancer-related programs
- Providing education before, during, and after treatment, including surgery preparation and recovery
- Assisting with referrals for genetic counseling, nutrition services, rehabilitation, lymphedema therapy, survivorship resources, and supportive care services
- Offering ongoing follow-up to monitor progress, address concerns, and ensure patient needs are being met
- Supporting informed decision-making by helping patients understand their care plan and available resources
- Providing emotional support and advocacy throughout the cancer experience
