Exercise is Medicine Online Application

Below you will find the link to the online application that you will need to complete prior to your first visit with an Exercise Physiologist. Upon completion, you will  receive instructions for the day of your appointment and directions to our facility.

Please set aside 15-30 minutes to complete the application. Once you start it, you must complete it or start over at another time. You will not be able to save your progress.
