What Is Exercise Is Medicine (EIM)?

EIM is a doctor-referred fitness program for patients with a chronic health condition or at high risk for developing a chronic condition, patients recently hospitalized due to their chronic condition, and patients who are not currently meeting the recommended exercise guidelines. EIM provides support for preventative and intervention care.

Who can participate?

Anyone with a doctor’s referral that can commit to attending three consultations with an Exercise Physiologist at our location in Atlanta.

What does the program include?

  • Two-week free membership to Piedmont Atlanta Fitness Center
  • Exercise Testing with norm comparison (when appropriate)
  • Individualized Exercise Prescription
  • Wellness Coaching
  • The opportunity to register for our 8-week Medical Fitness Program ($150)
  • The opportunity to join PAFC with waived initiation fee ($150 value)

How much does the program cost?

The program is free. There are options for additional services that are fee-based.

I am interested! How do I get a doctor’s referral?

Download and print the referral form and take it to your physician or e-mail it. Your physician will complete the form and fax it to us. We will contact you to schedule your first appointment.
