Atlanta, Ga. (August 13, 2025) – Piedmont has partnered with TimeDoc Health to deliver innovative care management services for patients with chronic conditions. This collaboration aims to improve patient engagement, outcomes, and access to care.
Care Connect encompasses comprehensive Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs and represents a significant step forward in Piedmont’s promise to champion patient-centered care for its diverse patient population.

TimeDoc Health’s scalable solution equips Piedmont with the tools and expertise needed to support seamless care coordination. The program integrates TimeDoc’s proprietary platform with Piedmont’s Epic EHR and existing workflows, and leverages data insights to deliver measurable results, empowering both patients and providers.

Piedmont’s Care Connect program focuses on reducing hospital admissions, improving chronic disease management, and enhancing patient satisfaction. The phased rollout will prioritize primary care, with plans to expand to specialty practices such as cardiology, OB/GYN, and endocrinology in the near future.
Once in the program, care coordinators check to make sure patients are monitoring their vitals at home, encourage medication adherence, assist with access to care, and provide an extra layer of encouragement and accountability for the patient to help them adhere to their prescribed plan of care.
