As a partner, you can help us connect our patients and team members to the resources and services made available through your organization. By using our collaborative platform, powered by FindHelp, together we can drive future initiatives and partnerships that are tailored to meet the needs of our local communities.

- Access to seamless reporting tools (this data helps support organizational business development goals)
- Ability to accept and process inbound referrals from Piedmont
- Manage and update your program listing on the platform
- Manage community appointments
- Create eligibility screeners
- Future collaboration and engagement opportunities
We have a commitment to ensure that patients get connected to resources. As an Empowered Partner, you’d be able to receive referrals directly from our Piedmont inpatient or ambulatory settings, ensuring a warm transfer of our patients into the community.
Getting Started with the Empowering You Platform
Add Your Organization and Programs
First, ensure that your organization and all applicable programs are listed on the platform. Search for your program.
If your program or organization is not listed, or if the information needs to be updated, please click the “Suggest a Program” button located at the bottom of the page.
Verify Your Information
If your program or organization is already listed and the information is accurate, please click "Claim Your Program."
This step confirms that your details are current and serves as a public-facing verification.
As a result, claimed organizations also appear higher in search results.
Start Receiving Referrals
Once your program is claimed and verified, you’re ready to receive referrals directly from Piedmont team members!
